With 16 facilities, we are able to assist with the needs of many types of residents. For more than 4 decades, we have worked together with our patients and their family members, physicians, staff and community volunteers to learn how to tailor programs that meet each person’s needs. In addition to registered nursing care, our staff includes licensed practical nurses and certified geriatric nursing assistants. Our medical directors are carefully selected based on experience and proven performance. We also specialize in comprehensive rehabilitative care administered by Continuum’s licensed physical, occupational and speech therapists. Our rehab experts provide patient-specific programs that are designed to achieve and maintain the resident's highest level of mobility, strength and independence, and we achieve these goals within an unparalleled 31-day average length of stay. 

All of our facilities accept Medicaid, Medicare, private pay and most private insurance, including Medicare Advantage plans.

Skilled Care

When people use the term skilled care, they are usually referring to some combination of rehabilitation and skilled nursing.

Many short-term care patients have complex medical conditions that require specialized or “skilled” nursing approaches and techniques. We hire nurses and nursing supervisors who have the experience and expertise to excel in providing skilled nursing care. Our nursing staff at each Heritage Hall facility is a permanent team. We make every effort not only to recruit a high-quality permanent team of local nurses, but we strive to keep them long-term.

As the premier provider of skilled nursing and rehab services in the state, Heritage Hall is committed to improving each resident's independence. Our services include acute, sub-acute and intermediate rehabilitation programs for the orthopedic, cardiovascular and neurological patients. Our interdisciplinary-team approach focuses the attention of all team members to achieve a positive rehab outcome.

Extended Care

We’re proud to say more than 80% of our patients return home after using our rehabilitation and skilled nursing services. But sometimes it’s not possible to receive all the care they need at home. That’s why Heritage Hall is dedicated to providing excellent extended care to patients requiring ongoing assistance with the routine activities of everyday life.

Everyone has different needs.

We know the importance of designing individualized plans to meet the emotional, physical and spiritual needs of our residents. Our care team works with the patient’s family and friends to develop a personalized care plan which includes different social activities, and personalized diet plans and therapies. It’s our goal for our residents to feel at home, which is why we encourage items like family photos, a favorite chair and other memorabilia to help the resident feel more comfortable. 

We encourage our extended care residents to participate fully in everyday routines. Residents dress for the day and come to the dining room for meals when possible. And mealtimes aren’t just about food: each meal is an event, a topic of discussion, and a time to socialize. Our numerous activities and therapies provide an important level of stimulation and socialization that our residents need. The encouragement and involvement of family is also vital to helping a new resident become fully socialized. 

We know that sometimes aging bodies need help remaining flexible and strong enough to maintain activity, which is especially important after illness or injury involving hospitalization. For that reason, extended care offers a special ongoing therapy program for eligible residents. Heritage Hall has over four decades of experience in providing compassionate, individualized extended care to Virginia’s aging family members, and our reputation is unmatched.

Memory Care

If you are caring for a loved one with advancing memory loss, Alzheimer’s or another form of dementia—we are here to help you. 

All Heritage Hall facilities have the training and security to meet the needs of patients with early or mild dementia. When the disease progresses to stages where a secured dementia unit becomes necessary, we will guide you through the transition to this specialized level of care.

Four of our Heritage Hall facilities can offer your loved one a secured home like environment. This carefully designed, secure environment has specially-trained staff, customized private activities, and offers dining spaces and a protected outdoor garden area. Our staff will work closely with your family to understand the needs of your loved one and we will create a program of care that is right for them.
